We became a second wave PMS Pilot in October 1999 and have a practice-based contract and since April 2010, we have become part of the City Hospital Sunderland Foundation Trust following the integrated pilot in care of the elderly to try improve services between primary and secondary care.
How to register
Any person living in our catchment area can register at the surgery. Please complete a form at reception including a new patient health questionnaire so that we can have some idea of your previous health and current medication. You will also be offered a health check appointment.

Ashbrooke, Thorney Close, Barnes, Grindon, St Chads and East Herrington
Provision for disabled patients
There is an extra-wide parking space with a ramp to the main entrace. An adapted toilet and the patient areas are on one level. There are extra wide entrance doors for wheelchair users and an induction loop for those patients who are hard of hearing. Our helpful reception team will ensure blind or partially sighted patients are catered for. If help is needed please ask for assistance.
We are an active research practice and we may pass on anonymised information. If we do, the information will not be able to be linked back to you in any way and will be used only for research purposes. For any further information, please contact the Practice Manager.
Confidentiality / Access to records
Every patient has a right to confidentiality (this includes under 16) and as a practice we adhere to the NHS Code of Practice and are compliant with the Access to Medical Records Act 1990.
There are certain services performed by GPs which are not covered by the NHS for which there may be a fee. These include: Employment Medicals, completion of insurance forms, fitness to drive certificates, private medical certificates etc. Please ask at reception and you will be informed if there is a fee. A list of charges based on the BMA recommended fees is displayed in the waiting room.
Accountable named GP
From 1 July 2015, all patients at the practice will have an accountable named GP – Dr Ashar Hussain.
The ambulance service is available only in cases of medical need. If you need an ambulance for a hospital appointment, your request must be made to our receptionist at least 2 working days before your appointment.
The averae pay for GPs working at Church View Medical Centre in the last financial year was before tax and National Insurance. This is for 2 full-time GPs and locum GPs who worked in the practice for more than six months.
GP Net Earnings
All GP practices are required to declare the mean earnings (e.g. average pay) for GPs working to deliver NHS services to patients at each practice.
The average pay GPs working at Church View Medical Centre in the last financial year was £57,029 before tax and National Insurance. This is for 2 full time GPs who worked in the practice for more than six months.
